We are an out of network provider so we do not have any arrangements or contracts with your insurance provider. As a courtesy we will file all insurance claims necessary for you. Additionally, we will work with you and your insurance provider to obtain as accurate a reimbursement estimate for you prior to beginning any treatment. While we do our best to carefully estimate the patient balance after factoring in what, if any, will be covered by your insurance, please remember that you are responsible for any remaining balance not covered by insurance. The estimated patient balance for professional services is due at the time such services are rendered.
If you have dental insurance and benefit coverage has been verified by our office, we will accept assignment and you will only be asked to pay the portion we estimate your insurance will not cover at each visit. If you have primary and secondary coverage, we are only able to take assignment from the primary carrier
We accept cash, check, MasterCard, Visa and Discover credit cards and we also offer CareCredit financing for treatment plans over $500.
Please be advised that the policy of this office is to charge $50.00 for a missed appointment unless canceled by contacting one of our staff 2 business workdays prior to their appointment. We have other patients on waiting lists and require adequate lead time to schedule them into any earlier times that may become available.
